Volunteer Information
Thank you for your interest in volunteering with the Fontana Unified School District (FUSD). Volunteers play a vital role in enriching the educational experience of our students and supporting our schools. Whether you are a parent, community member, or FUSD employee, your time and dedication are greatly appreciated.
To ensure a safe and productive environment for our students, we require all volunteers to complete the Volunteer Authorization Form This form helps us gather essential information and obtain all necessary clearances.
Sections to Complete:
- Volunteers: Fill out personal information, select the type of volunteer assignment, and provide any necessary explanations.
Once you have completed the form, please return it to the school site. Your cooperation helps us maintain a secure and supportive atmosphere for everyone involved.
Types of Volunteering:
- Standard Volunteer: Short-term, under direct supervision (no fingerprints/TB test required).
- Preschool Volunteer: Requires immunizations (TB, TDAP, MMR, optional Flu).
- School Activity Volunteer or Coaching: Requires fingerprints and Concussion Training Certificate (CTC). Coaching volunteers can speak with the athletic director for more details.
- Other Assignments Requiring Fingerprints: Include roles like tutoring, mentoring, transporting students, and any activity with potential unsupervised contact with children.
